Client Assessment

• Provide an initial on-site assessment to identify care needs
• Provide a Plan of Care to address care needs
• Arrange for needed services
• Manage and oversee services
• Coordinate services with other agencies
• Provide distant family liaison communication by alerting family or Power of Attorney when problems arise

Companion Services

• Companionship
• Meal preparation/assistance with eating
• Medication reminders
• Ambulation and exercise assistance
• Assistance with dressing
• Available by the hour (2 hour minimum)

Home Making Services

• Light housekeeping
• Laundry
• Changing linens

Transportation Services

• Shopping and errand assistance
• Transportation to doctors appointments, grocery store or the pharmacy
• Transportation to activities

Respite Services

• We provide short-term relief to the primary caregivers or family caregivers. Typically, we provide companionship, home making, meal preparation and bathing assistance while the primary caregiver is away.

Bath Aide Services

• Bathing of client
• Shampooing of hair
• Assistance with dressing

End of Life Care

In many cases, we provide supplemental staffing for clients under the care of a licensed hospice agency. We work with many hospice companies to provide continuous care.
